Before making a request for business cards, please read over the eligibility requirements listed below. If you are unsure if you fit the criteria, please consult with your supervisor. Then, if you qualify, fill-in the information in this form and submit for approval. Request for business cards will be honored if eligibility requirements are met.

You are only eligible for Children’s Guild business cards if:

You are an administrator, director, or organizational leadership
You represent The Children’s Guild or our affiliates in public venues
Your job entails making contacts with outside vendors, businesses, and/or donors
You meet with The Children’s Guild clients, families, or educational agency representatives
